Provider Credentialing can assist you with inquiries regarding new provider numbers, a change in tax identification numbers, change of address, changes with existing practices and adding new providers to a practice. All providers must have an individual provider identification number for each physical location on file before submitting claims for payment.
New providers should utilize the Uniform Provider Application located on our web site. This application may be completed online and submitted to Blue Cross and Blue Shield of Alabama. Click here for the Uniform Provider Application. When submitting an application electronically to Blue Cross, you will receive a confirmation e-mail with an identification number for tracking purposes and a list of additional forms and information requirements to complete the application. Please keep in mind that we will not be able to start processing your application until we receive all of the information that is requested in the confirmation e-mail. Any missing information can cause a delay in processing your application. If you do not receive a confirmation e-mail, contact us as soon as possible so we can give you a list of the additional information needed to complete your application. The completed application may be printed and used to send to other Alabama health care entities.
You may also obtain the application for a new provider number by calling Blue Cross and Blue Shield of Alabama’s Provider Data Department at 205 220-7528 or faxing 205 220-9545. Allow thirty days for the processing of your application. This provider number must be used on all of your patients’ claims to ensure payment is made to the proper provider of services. Accurate and complete information is important for both providers and Blue Cross. Blue Cross and Blue Shield of Alabama’ s provider file is utilized for remittance payments, Internal Revenue reporting and publication mailings.
