Provider Appeals
As of April 21, 2009, Blue Cross and Blue Shield of Alabama has an established appeals process for Providers and Physicians. The following documentation provides guidance regarding the process for appeals. A form is also available to aid providers in preparing an appeal request.
Post-Service Q&A Adobe version 7.0 or greater is required for using the appeal form. Click here to download the latest version of Adobe Reader. The form can be filled out online and printed, or printed as a blank document to be filled out by hand. When the form is filled out online, the form will automatically receive a barcode which can expedite the handling of your request.
