Blue Advantage Case Management

Blue Cross and Blue Shield of Alabama Blue Advantage Plan program is URAC accredited. Case management services are offered to all Blue Advantage and Special Needs Plan (SNP) members. Case management allows active decision-making opportunities for the patient, family/primary caregiver, and physician during the planning and provision of services for the patient's individualized and ongoing healthcare needs.

Blue Advantage case management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet a consumer's health needs through communication and available resources to promote quality, cost-effective outcomes. Our mission is to assist our members to improve their health through coordination of services and resources, providing education and collaboration with the healthcare team.
