Benefits of Enhanced Case Management:
- Access to a registered nurse care coordinator to research, assess, coordinate and monitor healthcare needs and provide information about community resources
- Assistance in addressing specialty services for individual needs, such as complex medical conditions, burns, high-risk pregnancy, pediatrics, organ transplant, physical rehabilitation, psychiatric and substance abuse
- Receive pre-surgical and post-surgical counseling to better understand what you should expect before, during, and after surgical procedures.
- Obtain information about using benefits effectively
If you are identified as someone who will benefit from this program, a registered nurse care coordinator will make outreach to invite you to participate.
Case Management is a voluntary program provided at no additional cost to Blue Cross and Blue Shield of Alabama members.
