What Can I Expect at My Annual Wellness Visit?

This appointment is designed to create a personalized prevention plan based on your current health. Your healthcare provider will ask you to fill out a health risk questionnaire and may also include:

  • A review of your medical and family history

  • Advanced Care Planning

  • Height, weight, blood pressure, and heart rate

  • Personalized health advice

  • A list of risk factors and treatment options for you

  • A screening checklist with preventive services personalized to your specific risks

  • Expect questions about fall risk

  • Does my family history put me at risk for certain conditions?

  • What is Advanced Care Planning?

  • Would you recommend any additional screenings?

In-home or Virtual Health Assessment

We want you to know that your health remains our top priority. That is why Blue Cross has partnered with Signify Health to offer in-home and telehealth assessments.

What Can I Expect at My In-Home / Virtual Check-up?

An in-home or telehealth assessment will be a little different than doing your annual wellness visit in your provider’s office. Clinicians can perform assessments either in your home or virtually through any of your own devices that have audio and video capabilities. During this type of assessment, you can expect:

  • A one-on-one appointment with a Signify clinician

  • No cost to you

  • The same level of focus and attention, but from the comfort of your own home

The Signify Health in-home or virtual visit should not replace your regular visit with your primary care provider (PCP) when you need care. You will receive a personalized checklist of your visit. Signify Health will send your PCP a summary of the visit.

Signify Health, LLC is an independent company providing health assessment services to eligible Blue Cross and Blue Shield of Alabama Members.
