Your annual wellness visit is an important opportunity to review your health history, get a physical exam, and discuss your health goals with your provider. During these visits, here’s what you can expect from your healthcare provider:

  • Health History Review: Review your medical history, medications, and any changes in your health since your last visit.
  • Physical Exam: A physical exam measuring your weight, height, and other vital signs.
  • Screenings and Tests: Based on your age and health risks, your provider may recommend blood pressure checks, cholesterol tests, or cancer screenings.
  • Discussion of Health Goals: Discuss your health goals, lifestyle changes, and any risk of falling.
  • Follow-Up Plans: If any issues are found, your provider will create a personalized care plan to address them.

What Can I Expect at My In‑Home Virtual Check-Up?

An in-home or virtual check-up is different from visiting your provider’s office for your annual wellness visit. You can have your assessment either at home or online using a device with audio and video. Here’s what you can expect:

  • A one-on-one appointment with a Signify clinician.
  • It’s free and won’t cost you anything.
  • You’ll receive the same level of care and attention, all from the comfort of your own home.

Blue Advantage® (PPO) is provided by Blue Cross and Blue Shield of Alabama, an independent licensee of the Blue Cross and Blue Shield Association.

The Signify Health in-home or virtual visit should not replace your regular visit with your primary care provider (PCP) when you need care. You will receive a personalized checklist of your visit. Signify Health will send your PCP a summary of the visit.
