A social worker can help you find community resources, provide information, and refer you to programs and agencies that may benefit you.

Your social worker will also work with healthcare professionals, public aid organizations, and community resources to obtain and coordinate services.

Your Blue Cross Social Worker can help you identify essential resources such as:

  • Providers who can modify your home by installing wheelchair ramps, handrails, grab bars, and offering weatherization services, minor home repairs, and other assistance
  • Financial assistance to help with your utility, rent, and mortgage expenses
  • Personal care services that offer bathing and grooming assistance
  • Relief care services to support your caregiver
  • Housekeeping services for light cleaning, vacuuming, and cooking
  • Community and corporate resources to assist with the cost of expensive medications
